WHAT TO BRING TO YOUR VISIT(S)
Insurance card(s) & any copay/specialist copay (if applicable)—they are required at each visit.
Prescription card(s) (if applicable)—they are required at each visit.
If you do not have insurance, payment for your visit is due at the time of service.
A list of all medications and daily dosage(s) you are currently taking (including over-the-counter remedies, vitamins, and herbal supplements).
Please feel free to bring a list of questions or concerns about your condition/treatment to any office visit.
If a minor is unable to be accompanied by a parent or guardian, please contact the office for further direction.
If you need to cancel or reschedule an appointment, please notify the office at least 24 hours prior to the appointment. There will be a cancellation fee of FIFTY DOLLARS ($50) for appointments canceled with less than 24-hour advance notice.

Patient Portal
The patient portal gives you access to the online services offered by our practice with the assurance that all of your information is encrypted and stored securely. Simply click on the link to access patient registration, medical records, billing and more!
Prior to your visit(s), Complete/Update the following information within your patient portal on myDashboard: Patient Information, Insurance Information, and any forms that have been added.
FINANCIAL AGREEMENTS
Please take a moment to review our financial policy(s) so that you understand your responsibility regarding the charges for the services rendered to you by this office. You will be asked to sign electronically stating you have read and understand at the time of check-in.
HIPAA | Notice of Privacy Practices
Our HIPAA Notice of Privacy Practices provides information about how Three Rivers Dermatology & Windy Ridge Skin Care Centre may use and disclose protected health information to carry out treatment, payment and healthcare operations. It also describes your rights to access and control your protected health information. We have provided a copy for you to review.

From Downtown | Take US-24 West/ West Jefferson Boulevard west towards Huntington. Pass Jefferson Pointe Mall, Lutheran Hospital, and I-69. Turn right at the light onto Coventry Lane*…
…*follow Coventry Lane around and make a U-turn in front of the Allen County Public Library - Aboite Branch. After turning, our practice is the next building on the right. We are on the backside of the building.
